Fountain Transitional Housing is proud to partner with the Healthy Alliance Social Care Network (SCN) to help eligible individuals and families access health-related social needs (HRSN) services that improve overall health and well-being.

The Social Care Network is designed to address the non-medical factors that impact health, including housing, food security, transportation, and other essential needs. By connecting participants with community-based services, we help reduce barriers that often prevent individuals and families from achieving stability and better health outcomes.

Our Services

As a Social Care Network partner, we assist eligible participants by providing or coordinating access to services such as:

• Housing navigation and housing stabilization

• Transitional housing support

• Health-Related Social Needs (HRSN) screenings

• Enhanced HRSN Care Management referrals

• Food and nutrition resources

• Non-medical transportation coordination

• Assistance with benefit applications and community resources

• Referrals to healthcare providers and behavioral health services

• Employment and workforce development resources

• Financial literacy and budgeting support

Who May Be Eligible?

​

Services may be available to individuals enrolled in participating Medicaid Managed Care Plans who meet eligibility requirements established by the Social Care Network and New York State. Eligibility is determined through a screening and assessment process.

Our experienced team works closely with healthcare providers, hospitals, managed care organizations, social service agencies, and community partners to ensure participants receive coordinated, person-centered support.
